The Mt. Diablo Unified School District would like to notify you of a new immunization requirement for students entering the 7th through 12th grade. Students entering these grade levels are now required to receive a booster shot against pertussis (also known as Tdap). Assembly Bill 354, now chaptered into California law, requires students to be immunized against pertussis.

Pertussis is widespread. If your child is 7 years old or older and has not received the 'Tdap' pertussis booster shot, please contact your doctor or health department. Getting a booster show now will protect your child against the ongoing threat of pertussis and meet a new school requirement.

Beginning July 1, 2012, and beyond, all students entering 7th grade will need proof of a Tdap booster shot before starting school.

If your child has already received a Tdap booster, please bring proof to your child's school. If your child has not received a booster shot, please obtain it as soon as possible. Proof must be submitted before the start of the school year or the student will not be allowed to attend.

If you currently have a waiver in place it needs to be replaced by the new State wavier which will need to be signed by you and your student's physician.

New Documentation Requirement for Personal Belief Exemptions:

Starting in 2014, a new California law will modify the process for obtaining exemptions to student immunizations based on personal beliefs. 

Assembly Bill (AB) 2109 requires documentation that health care practitioners have informed parents about vaccines and diseases.

Form CDPH 8262| Spanish, fulfills California requirements (California Health and Safety Code §120365) for a parent or guardian seeking to obtain an exemption because of personal beliefs to one or more immunizations required for child care or school. You may also download and print an image of the form in English and Spanish.
