The Home & Hospital school is a temporary program designed to help maintain a student’s academic standing during a time of illness or other temporary disability. Students are entitled to a maximum of 5 hours per week of home instruction. The teacher will be there at times agreed upon and at a minimum of up to 1 hour per day. No sessions are allowed on holidays or weekends. A responsible adult (18 years or older) must be present when the teacher is in the home.
The purpose of Home & Hospital is to support the student in maintaining a continuity of instruction during the student’s absence from the regular program.
Parents may apply for home instruction the first day their child is absent from school. Students must have a medical or psychological condition, verified by a doctor’s signed authorization which requires the need for home instruction for a minimum of five days from the date of the doctor’s authorization.

This form must be completed and signed by a California licensed physician and include a medical diagnosis. Page 1 of the form is filled out by the parent or school and signed by the Administrator at the home school. The parent must sign the Medical Release at the bottom of page 1. The physician must complete Page 2 of the form. General education students in grades K-12 may be eligible for service if: they have a medically verified, non-contagious physical condition(s) or a temporary physical disability that cannot be accommodated at their home school. If your student has an IEP, a change of placement meeting must be held and a copy of the addendum and meeting notes must accompany this application.

This form must be completed and signed by a California licensed psychiatrist and include a diagnosis. This form must be completed by the Program Specialist and may include, but are not limited to the following circumstances: IEP must be held to reflect change of placement to Home & Hospital.
- Pending initial assessment for special education services
- Pending District placement
- Pending initial placement in a non-public school
- Pending reassignment from one non-public school to another
- Pending residential placement
- Pending a District IEP meeting when safety on campus is an issue
- Pending a pre-expulsion IEP meeting or expulsion hearing
HOME & HOSPITAL FORMS ARE ONLY VALID FOR THE CURRENT SCHOOL YEAR. IF SERVICES ARE TO EXTEND INTO THE FOLLOWING SCHOOL YEAR, A NEW APPLICATION IS REQUIRED.

Once you are cleared to work in MDUSD, please download the two Home & Hospital forms: Teacher Information and Teacher Consent. Fax the completed forms to our office. Once you are cleared to work in Home & Hospital, the Administrator will contact you for an interview and orientation.
